Requesting Repeat Prescriptions:

Aged 16 or over? We are no longer accepting requests via the current online form for patients aged 16 years and over. If you are 16 years or over you can now request your repeat prescription either via the new online service "Patient Access", see below for more information, or alternatively you can use the right-hand side of your last prescription by ticking the items you need and dropping the request into the surgery.

Aged under 16? You can continue to request repeat medication via the online form. 

Please note that repeat prescriptions cannot be processed over the phone or by email.
 

New online service 'Patient Access'   

This is a new service that you will need to register for. Not only will you be able to see what medication you are on but it also allows you to request repeat prescriptions at the click of a button. You will also be able to book/cancel online appointments, amend your contact details, view any allergies you may have and see your immunisation status. If you haven't already registered for this service you will need to come, in person, to the surgery and bring along 2 separate forms of identification (1 form of photo ID and 1 separate item showing proof of address) and complete the Patient Access Registration form. You will then be given your login details with instructions on how to register.

Examples of acceptable ID include a driving licence, passport, a recent utility bill (e.g. gas, electric, satellite, TV, phone bill) issued within the last 3 months, a bank statement, council tax bill for the current council tax year, mortgage statement.

This service is not available for children aged under 16. For more information on how Patient Access works please visit patient.emisaccess.co.uk.



Electronic Prescribing Service (EPS)   
 
Since February 2015, this practice has been using the Electronic Prescribing Service (EPS). This means that your prescription will go electronically to your nominated pharmacy. If you haven't already nominated a pharmacy please let us know so that we can set this up for you, otherwise you will have to collect your paper prescription from reception.

If ordering online, please use the free text box to tell us which pharmacy you would like your electronic prescription to go to.

Your prescription will be ready to collect from your nominated pharmacy (or the Practice if you haven't nominated a pharmacy) after 3 full working days.

We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you may be asked to make an appointment.
